ADDAX HOME HEALTH LLC in Fort Myers, FL is seeking PRN Physical Therapists and Physical Therapist Assistants to deliver in-home care. Responsibilities include evaluating patients in their homes, developing individualized treatment plans, providing therapeutic exercises, and documenting progress.

Qualifications

Responsibilities

• Conduct comprehensive PT evaluations and assessments in patients' homes
• Develop and implement individualized treatment plans to restore function and mobility
• Provide therapeutic exercises, manual therapy, and rehabilitative interventions
• Monitor and document patient progress, adjusting care plans as needed
• Collaborate with physicians, nurses, and other healthcare professionals for coordinated care
• Educate patients and caregivers on home exercise programs and injury prevention
• Maintain accurate and timely clinical documentation in compliance with regulatory standards

About the Role

1. ADDAX HOME HEALTH LLC is looking for PRN passionate and skilled Physical Therapists/Physical Therapists Assistants to join our growing team in Fort Myers, FL. Responsibilities
