Job summary

Sandgate Gardens Rehab and Nursing Center is seeking a skilled Physical Therapist to join our dedicated team in a full-time role. You will assess patients, design individualized treatment plans, and implement evidence-based therapies to reduce pain and improve mobility.

We seek a licensed PT with strong communication and teamwork skills, capable of working both independently and within a collaborative clinical team.

Qualifications

• Master/Doctorate in Physical Therapy from an accredited program.
• Successful completion of the National Certification Examination.
• Valid Physical Therapist license in the practicing state.

Responsibilities

• Assess patients' medical histories and current physical conditions.
• Diagnose movement patterns by observing patients and listening to concerns.
• Develop personalized treatment plans tailored to each patient’s needs.
• Establish clear goals and expected outcomes for each treatment plan.
• Utilize exercises, stretching, equipment, and hands-on therapy to reduce pain and improve mobility.
• Educate patients and families on the recovery process and self-care techniques.
