Caringhand Home Health Solutions, Inc is seeking a Physical Therapist to provide skilled services in patients' homes across California. This field-based, per diem role requires travel to patient residences and is listed as remote but not telehealth.

Responsibilities include evaluating mobility, delivering therapy per plans, educating families, and collaborating with the care team. Opportunities exist to convert to full-time based on performance.

Formación

• Active California Physical Therapy license.
• Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Minimum 2 years of PT experience; home health background is a plus.
• Current CPR certification with AHA.
• Valid driver's license and insured vehicle.
• Flexible Schedule.

Responsabilidades

• Evaluate patients' functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician's plan of care and California's Physical Therapy Practice Act.
• Educate patients and caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team; report changes in patient condition and participate in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.

This is a field-based, per diem position that requires travel to patients' homes. Although listed as "remote," this is not a work-from-home or telehealth role. Compensation is $160-$180 per visit.

• Strong communication skills and the ability to connect with patients and families.

• Current CPR certification with AHA.

• Valid driver's license and reliable, insured vehicle.
