Job summary

Just-Be-Fit, Inc. Physical Therapy is seeking a dedicated Physical Therapist to join our outpatient clinic in Deerfield, IL.

The position can be part-time or full-time, offering flexible scheduling to accommodate patient care needs. You will evaluate patients, determine diagnoses and develop individualized treatment plans, while establishing functional goals based on clinical examination and upholding HIPAA compliance.

Qualifications

• Bachelor's, Masters or Doctorate Degree from an accredited Physical Therapy Program.
• Current State of Illinois Physical Therapy license.
• CPR Certified.

• Provide continuous communication with referring physicians and other referral sources to ensure a consistent level of care.
• Maintain patient charts, clinical logs and checklists necessary for compliance.
• Maintain patient confidentiality with regard to medical records per company policies and procedures and assures compliance with all HIPAA guidelines.
