SCHOFIELD&ASSOCIATES LLC in Indiana is seeking a Home Health Physical Therapist (PT) to support Johnson & Bartholomew Counties. The PT will evaluate patients in their homes, develop individualized treatment plans, and provide skilled therapy one-on-one to improve strength, balance, mobility, and independence.

Ideal candidates hold a current Indiana PT license and demonstrate strong clinical assessment and communication skills, with reliable transportation.

Qualifications

• Current Indiana Physical Therapist (PT) license.
• Home Health experience preferred, but not required.
• Strong clinical assessment and communication skills.
• Compassionate, dependable, and patient‑focused.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide skilled physical therapy services in patients’ homes.
• Help patients improve strength, balance, mobility, endurance, and overall function.
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques.
• Monitor patient progress and modify treatment plans as needed.
• Complete accurate and timely documentation.
• Collaborate with physicians, nurses, and the interdisciplinary Home Health team to provide exceptional patient care.
