Job summary

Vitality Health Care LLC is seeking a compassionate Mobile Physical Therapist to join our in-home rehabilitation team in Essex/Passaic County, NJ. You will provide one-on-one care in patients' homes, helping them regain mobility and independence.

Responsibilities include evaluations, personalized treatment plans, gait/balance training, home exercise instruction, documentation, and coordination with physicians. A NJ license, DPT, and CPR are required.

Qualifications

• Doctor of Physical Therapy (DPT) from an accredited program.
• Current New Jersey Physical Therapist license in good standing.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Strong clinical reasoning and independent decision-making skills.
• Excellent communication and interpersonal skills.
• Experience in home health, outpatient orthopedics, geriatrics, or neurological rehabilitation preferred.

Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop individualized, evidence-based treatment plans based on goals and functional abilities.
• Provide skilled interventions to improve strength, balance, mobility, endurance, flexibility, and overall function.
• Educate patients and caregivers on home exercise programs, fall prevention, and safe use of assistive devices.
• Regularly reassess patient progress and modify treatment plans to maximize outcomes.
• Complete timely documentation including evaluations, daily notes, progress reports, and discharge summaries.
• Collaborate with physicians and other professionals to coordinate high-quality care.

What You'll Do

• Build trusting relationships with patients and families through compassionate, patient-centered care.
• Maintain compliance with Medicare Part B regulations, state practice acts, and agency policies.
• Stay current with evidence-based practice through continuing education and professional development.

Qualifications

• Current New Jersey Physical Therapist license in good standing.
