Job summary

Blue Stone Therapy is actively seeking a Physical Therapist (PT) to provide comprehensive rehabilitation services at Pinnacle Specialty Care in Cedar Falls, IA. You will evaluate adult and geriatric patients and develop individualized plans of care, delivering targeted PT interventions to improve gait, balance, mobility, and functional independence.

The role emphasizes collaboration with an interdisciplinary team, timely documentation, and adherence to professional standards.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license (or eligibility) in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong clinical reasoning, communication, and organizational skills.
• Commitment to patient-centered care and professional standards.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide skilled PT treatment focused on functional and measurable outcomes.
• Address gait training, balance, mobility, strength, range of motion, pain, and discharge planning.
• Supervise Physical Therapy Assistants in accordance with licensure and regulatory standards.
• Maintain appropriate patient schedules per established guidelines.
• Complete timely documentation to support quality care and reimbursement requirements.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.

Blue Stone Therapy is seeking a Physical Therapist (PT) to provide rehabilitation services to adult and geriatric patients at Pinnacle Specialty Care, Cedar Falls, IA.

This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Physical Therapist is responsible for patient assessments, care plan development, and implementation of treatment programs designed to improve gait, balance, mobility, strength, range of motion, pain management, and overall functional independence.
