Job summary

Pinnacle Health & Rehab at Canton is seeking a Physical Therapist to join our premier skilled nursing and residential care facility in Canton. You will plan, develop, and direct physical therapy services to ensure high-quality resident care.

The role may be part-time or full-time and requires Maine licensure as a Physical Therapist. You will work with a multidisciplinary team, supervise assistants, and contribute to discharge planning and individualized care plans.

Qualifications

  • Graduate of an accredited School of Physical Therapy.
  • Valid Maine Physical Therapist license.
  • Ability to work in a fast-paced environment with professionalism.

Responsibilities

  • Plan, develop, and implement physical therapy services for residents.
  • Maintain documentation and department records.
  • Communicate PT programs with the care team.
  • Participate in discharge planning and resident care plans.

About the Role:

We are seeking a highly qualified and motivated Physical Therapist to join our team on a PART_TIME_AND_FULL_TIME basis. As a Physical Therapist at Pinnacle, you will be responsible for:

  • Planning, developing, and implementing physical therapy services to meet the needs of our residents
  • Maintaining accurate documentation, department records, and reports
  • Ensuring effective communication regarding PT programs and collaborating with other healthcare professionals
  • Participating in discharge planning and development of resident care plans
  • Supervising and delegating tasks to Physical Therapy Assistants
  • Performing evaluations and screens in a timely and efficient manner
Requirements:

To be considered for this role, you must:

  • Be a graduate of an accredited school of Physical Therapy
  • Hold a valid license as a Physical Therapist in the state of Maine
  • Demonstrate the ability to work in a fast-paced environment and maintain a high level of professionalism
