Job summary

Canyon Homecare & Hospice in Utah seeks a dedicated Physical Therapist to join our home health therapy team, delivering skilled PT in patients' homes to improve mobility and independence.

You’ll perform evaluations, design individualized plans, educate caregivers, and document care in line with Medicare and company standards while coordinating with the interdisciplinary team. Full-time or part-time options available, with flexible scheduling.

Qualifications

• Current, unrestricted Physical Therapist license in Utah.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.
• Strong clinical reasoning and communication skills.
• Ability to travel between patient homes.
• Home health experience preferred.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide skilled therapeutic interventions to improve strength, balance, mobility, coordination, endurance, and functional independence.
• Treat patients with orthopedic, neurological, cardiopulmonary, and medically complex conditions.
• Educate patients and caregivers regarding home exercise programs, safety, fall prevention, and maximizing independence.
• Communicate effectively with physicians, nurses, therapists, caregivers, and the interdisciplinary team.
• Complete accurate and timely clinical documentation per Medicare, regulatory, and company requirements.
• Participate in care coordination and reassessment of patient progress.
• Supervise and collaborate with Physical Therapist Assistants per regulations and patient’s plan of care.

Canyon Homecare & Hospice is seeking a motivated and compassionate Physical Therapist (PT) to join our growing home health therapy team.

As a Home Health Physical Therapist, you will provide skilled physical therapy services to patients in the comfort of their own homes, helping them improve mobility, regain independence, and achieve their functional goals.

What You’ll Do

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide skilled therapeutic interventions to improve strength, balance, mobility, coordination, endurance, and functional independence.
• Treat patients with a variety of orthopedic, neurological, cardiopulmonary, and medically complex conditions.
• Educate patients and caregivers regarding home exercise programs, safety, fall prevention, and strategies for maximizing independence.
• Communicate effectively with physicians, nurses, therapists, caregivers, and other members of the interdisciplinary team.
• Complete accurate and timely clinical documentation in accordance with Medicare, regulatory, and company requirements.
• Participate in care coordination and reassessment of patient progress.
• Supervise and collaborate with Physical Therapist Assistants in accordance with applicable regulations and the patient’s plan of care.

What We’re Looking For

• Current, unrestricted Physical Therapist license in the state of Utah.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.
• Strong clinical reasoning and communication skills.
• Ability to work independently and manage a flexible schedule.
• Previous home health experience is preferred but not required. We are willing to train the right candidate.
• Reliable transportation and the ability to travel between patient homes.
