Job summary

Hilltop Lodge Healthcare & Rehabilitation is seeking a Physical Therapist to develop an individualized therapy program, perform assessments, and implement plans to maximize functional progress. You will coordinate with a multidisciplinary team and communicate treatment progress to patients and staff.

The role requires Kansas licensure, graduation from an accredited PT program, and strong documentation and communication skills. This position emphasizes quality patient care within a SNF setting.

Qualifications

  • Graduate of an accredited PT program (Bachelors, Masters, or Doctorate) in Physical Therapy.
  • Proficiency with all applicable state, federal, and professional standards.
  • Current license from the state Board of Physical Therapy Examiners.

  • Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
  • Designs and implements a plan of care for patients based on a thorough assessment and involves significant others when appropriate.
  • Conducts therapy practice within defined standards of care.
  • Directs the assistance of support staff to achieve patients’ functional goals.
  • Communicates effectively with other professionals and support staff to improve patient outcomes.
  • Complies with facility and departmental policies, safety, and infection control standards.
  • Documents evaluations, goals, and plans; maintains up-to-date documentation and signatures.
  • Keeps current with professional literature and participates in continuing education.

Job Summary:

The Physical Therapist is responsible for the development of an individualized therapy program consisting of physical assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff.

Duties/Responsibilities:
  • Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients’ functional needs and adjusts care plans to ensure optimal patient outcomes.
  • Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.
  • Conducts therapy practice within defined standards of care.
  • Directs the assistance of support staff to achieve patients’ functional goals.
  • Communicates effectively with other professional and support staff to achieve positive patient outcomes.
  • Practices facility and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.
  • Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures.
  • Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on professional literature.

Education and Experience:
  • Graduate of an accredited Bachelors, Masters, or Doctorate in Physical Therapy.
  • Proficiency with all applicable state, federal, and professional standards
  • Current license from the state Board of Physical Therapy Examiners.
Physical Requirements:
  • Duties require extensive standing and walking.
  • Requires lifting, positioning, pushing, and/or transferring patients.
  • Duties require frequent reaching, stooping, bending, kneeling, and crouching.
