Job summary

Alphacare ATX Health Solutions is seeking an experienced Physical Therapist for PRN home health visits throughout the Austin area. You will evaluate and treat patients in their homes to improve safety and independence, and collaborate with physicians and nurses to coordinate care.

The ideal candidate holds an active Texas PT license, has home health experience, and can work independently with strong documentation skills using Kinnser/WellSky EMR. CPR certification is required.

Qualifications

• Active Texas Physical Therapist license required.
• Home health experience is required.
• CPR certification required.
• Experience with Kinnser/WellSky preferred.
• Reliable transportation and valid driver's license.
• Strong communication and documentation skills; ability to work independently.

Responsibilities

• Evaluate and treat patients in their homes to improve safety, independence, and functional performance with daily activities.
• Develop individualized treatment plans and home exercise programs.
• Assess and recommend adaptive equipment, home modifications, and fall-prevention strategies.
• Collaborate with physicians, nurses, OTs, and other healthcare professionals.
• Educate patients and caregivers on functional mobility, self-care, and safety techniques.
• Complete timely and accurate documentation in Kinnser/WellSky.
