Job summary

Vista Ridge Senior Care in Lakeway, TX is seeking a licensed Physical Therapist to develop and implement the plan of care for facility residents and short-term patients. You will supervise PT Assistants, conduct screenings, and collaborate with an interdisciplinary team to advance patient outcomes.

The role supports an in-house therapy team, new equipment, and a focus on efficient patient time. Full-time schedule with CEU/licensure reimbursements and a commitment to a family-like atmosphere.

Responsibilities

• Develop and implement the PT plan of care for residents and short-term patients.
• Provide supervision for PT Assistants delivering the PT plans of care.
• Assist with patient screenings for PT needs.
• Communicate progress and barriers to PT goals with the interdisciplinary team.
• Complete timely PT documentation in compliance with regulations.
• Maintain rehab gym equipment and treatment area with infection control.
• Provide training and education to patients, families, and caregivers regarding the PT plan of care.

This position is for a skilled nursing facility in Lakeway, which has both short-term and long-term patients. Vista Ridge Senior Care has an in-house therapy team with smaller daily case loads, all new equipment, and an emphasis on optimizing time with patients.

Physical Therapist Job Responsibilities

• Develop and implement the Physical Therapy plan of care for facility residents and short-term patients.
• Provide supervision, as needed, for staff Physical Therapy Assistants in delivery of the PT plans of care.
• Assist as needed with patient screenings for the need for Physical Therapy.
• Communicate progress and any barriers toward PT goals with interdisciplinary team members.
• Complete timely PT documentation in compliance with regulatory, licensing, and payor requirements.
• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
• Provide and document patient, family, and caregiver training regarding each PT patient’s specific plan of care and recommendations for follow up.
