Job summary

Applesoft Homecare Services LLC in Dallas–Fort Worth is seeking a licensed Physical Therapist to deliver skilled PT in patients’ homes, focusing on restoring function and mobility. You will conduct evaluations, develop individualized care plans, and provide interventions including therapeutic exercises, gait and balance training, and safety education.

The role requires coordination with physicians and the interdisciplinary team, plus compliant documentation.

Qualifications

• Current Texas Physical Therapist license in good standing
• Graduate of an accredited Physical Therapy program
• Minimum 1 year of experience preferred (home health experience strongly preferred)
• Knowledge of Medicare home health regulations
• Valid Texas driver’s license, reliable transportation, and auto insurance
• Current CPR/BLS certification
• Ability to work independently and manage a mobile caseload

Responsibilities

• Perform comprehensive physical therapy evaluations in the home setting
• Develop, implement, and update individualized Plans of Care (POC) in collaboration with the physician
• Provide skilled interventions including: Therapeutic exercise; Gait and balance training; Transfer and mobility training; Pain management techniques; Fall-prevention strategies
• Evaluate patient response to treatment and modify treatment plans as clinically indicated
• Educate patients and caregivers on: Home exercise programs; Safety techniques; Disease processes and functional limitations
• Complete timely, accurate, and compliant documentation per: Medicare Conditions of Participation; Texas HHSC standards; Agency policy
• Participate in interdisciplinary team communication and case conferences
• Coordinate care with physicians, nursing staff, and other disciplines
• Supervise Physical Therapy Assistants (PTAs), if applicable, in accordance with Texas regulations
• Adhere to infection control, patient confidentiality (HIPAA), and safety standards
• Perform discharge planning when therapy goals are met or services are no longer medically necessary

We are seeking a licensed Physical Therapist who is clinically skilled, documentation-savvy, and passionate about home-based care.

Position Summary

The Physical Therapist provides skilled physical therapy services to homebound patients in accordance with the physician’s plan of care, applicable state and federal regulations, and agency policies. Services focus on restoring function, improving mobility, reducing pain, and preventing disability.

Essential Duties & Responsibilities

The Physical Therapist will:

• Perform comprehensive physical therapy evaluations in the home setting

• Ability to stand, walk, bend, and perform therapeutic activities
• Ability to make independent clinical decisions within scope of practice
