Job summary

Northshore Home Health Care is seeking a full-time Physical Therapist to join our inpatient rehabilitation team. You will evaluate and treat patients recovering from major illness, injury, or surgery, and develop evidence-based care plans to restore function and independence.

Collaborating with physicians, nurses, and therapists across disciplines, you will also provide education to patients and families and participate in care conferences and discharge planning.
