Job summary

Intermountain Health is seeking a fully remote Pharmacy Technician to support population health initiatives, optimize medication use, and improve patient outcomes. You will work with pharmacists and care teams to coordinate refills, outreach, and barrier identification, with a focus on adherence and continuity of therapy.

Preferred candidates have Medicare Stars experience. The role requires Colorado licensure, remote work eligibility, and a Monday–Friday schedule with a 40-hour work week.

Qualifications

  • Completion of pharmacy technician training program or at least two (2) years of experience as a pharmacy technician.
  • Current pharmacy technician certification and/or license in state of practice.

Responsibilities

  • Collaborates with pharmacists and other healthcare professionals to manage medications and improve adherence.
  • Supports interventions to improve health outcomes and reduce costs within value-based care.
  • Assists in medication reconciliation and ensuring accurate medication lists.
  • Contacts patients to improve adherence and addresses barriers to taking medications.
  • Collects and analyzes data related to medication use, adherence, and outcomes.
  • Coordinates care and ensures patients receive appropriate medication-related services.
  • Educates patients and families about medications, side effects, and adherence.

Schedule: Monday through Friday, no weekends or holidays.

Shift: 8:00 am to 4:30 pm MST

Location: This is a fully remote position. Candidates for this role need to reside in and have an active Pharmacy Technician license in Colorado. Candidates outside of this state will not be considered.

Job Function

The Pharmacy Technician supports population health initiatives by optimizing medication use and improving patient outcomes within a specific group or community.

Essential Functions
  • Collaborates with pharmacists and other healthcare professionals to manage medications, improve adherence, and address medication-related issues that impact the overall health of a population.
  • Plays a crucial part in value-based care programs by monitoring medication adherence, identifying gaps in care, and supporting interventions aimed at improving health outcomes and reducing healthcare costs.
  • Assists in medication reconciliation, ensuring patients have accurate medication lists, especially during transitions of care.
  • Works to improve medication adherence by contacting patients, providing education, and addressing barriers to taking medications as prescribed.
  • Collects and analyzes data related to medication use, adherence, and outcomes to identify trends and areas for improvement within the population.
  • Collaborates with pharmacists, physicians, nurses, and other healthcare professionals to coordinate care and ensure patients receive appropriate medication-related services.
  • Educates patients and their families about medications, potential side effects, and the importance of adherence, often through phone calls or other communication methods.
  • Contributes to population health initiatives that focus on improving quality of care, reducing costs, and meeting performance metrics within value-based care models.
  • Generates reports on medication adherence, therapeutic outcomes, and other relevant metrics to track progress and identify areas for improvement.

Minimum Qualifications
  • Completion of pharmacy technician training program or at least two (2) years of experience as a pharmacy technician.

  • Outpatient or Retail experience
Physical Requirements
  • Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
  • May be expected to stand in a stationary position for an extended period of time.
  • For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
