Job summary

UMass Memorial Health is seeking a Pharmacy Technician to support transition of care under pharmacist supervision. You will collect and verify allergy and medication histories and communicate with patients, families, and the care team to ensure safe medication transitions upon hospital admission.

Under the supervision of a pharmacist, you will operate EHR systems, coordinate with pharmacies and clinics, and participate in a multidisciplinary TOC team to optimize patient care.

Qualifications

  • License/Certification/Education: High school diploma or equivalent required.
  • Massachusetts pharmacy technician registration and national pharmacy certification (PTCB) required.

Responsibilities

  • Provide hands-on total quality patient care with pharmacists and care teams.
  • Obtain medication and allergy history from patients and families.
  • Interview patients and families.
  • Use active listening.
  • Overcome barriers including language barriers.
  • Understand basic medication information.
  • Collaborate with ED admitting teams and pharmacists.
  • Access EHRs to obtain med history.
  • Contact external facilities for med history.
  • Coordinate with pharmacies, physician offices, hospitals and long-term care facilities.

Schedule Details: Monday through Friday, Saturday Scheduled Hours: Day, Future Saturday only weekend rotation. Under the direct supervision of a pharmacist, performs transition of care (TOC) related tasks such as collection and verification of allergy and medication history.

The TOC liaison will communicate with patients, families, nurses, physicians, and other healthcare professionals to obtain accurate home medication lists upon hospital admission.

Provides integrated support for patients and the hospital’s admitting team to ensure a safe medication transition by providing a "hands-on" approach to total quality patient care.

Major Responsibilities
  • Provides integrated support to patients, pharmacists, and various healthcare providers by providing a "hands-on" approach to total quality patient care.
  • Works closely with patients and/or their families to obtain the best possible medication and allergy history.
  • Interviewing patients and/or their families
  • Utilizing active listening skills
  • Utilizing available resources to overcome barriers (i.e. language barriers)
  • Understanding basic medication information
  • Integrate and collaborate with the emergency department’s admitting clinical teams, consisting of physicians, nurse practitioners, nurses, and case managers (i.e. Transition of Care Team, ED Team, Medicine Team, Surgery Team,etc.).
  • Operate electronic health records to obtain patient medication history information.
  • Contact outside medical facilities to obtain patient medication history information.

Position Qualifications
  • License/Certification/Education: Required: High school diploma or equivalent required.
  • Massachusetts pharmacy technician registration and national pharmacy certification by Pharmacy Technician Certification Board (PTCB) required.
Experience/Skills
  • Required: Three years pharmacy experience required, with both community and inpatient hospital experience preferred.
  • Must be proficient in math with knowledge and ability to use a computer for data entry.
  • Must be able to follow oral and written instructions with accuracy, efficiency, and dependability.
