Job summary

Northern Dutchess Hospital in Rhinebeck, NY is hiring a Medication History Technician to obtain and document complete home medication histories, including prescription, OTC and herbal products, for accurate EMR entry. Under the supervision of a pharmacist, you will verify allergies, respond to requests from nurses and physicians, coordinate follow-up, and ensure compliance with med history processes.

A high school diploma and at least 1 year of experience are required; PTCB and NY pharmacy

Qualifications

• High School Diploma or equivalent.
• Minimum Experience: 1 year.
• PTCB Certification within 6 months.
• License: Registered Pharmacy Technician with NY Board of Pharmacy within 6 months.

Responsibilities

• Obtains and clarifies patient's home medication list, including prescription, OTC, and herbal products.
• Responds to nurses, physicians, pharmacists, or other requests to manage, triage appropriately, and ensure follow-up regarding med history information.
• Obtains patient's allergies & intolerances and documents in EMR with symptoms, reactions, and severity.
• Directs, triages, and communicates questions from patient/family/care partner outside scope of MHT.
• Identifies and participates in quality assurance to obtain an accurate med history list.
• Fulfills all compliance responsibilities related to the position.
• Models Nuvance Health Values and maintains attendance.
• Performs other duties as required.

Purpose

Under the supervision of a pharmacist, the medication history technician (MHT) will obtain medication histories through interviews with the patient/family/care partner. The MHT will use technology to pull in external fill history from patients' pharmacies.

Essential Responsibilities

1. Obtains and clarifies patient's home medication list, including prescription, OTC, and herbal products. Any bottles brought in will be promptly returned to the patient's family/care partner after all important information has been transcribed.
2. Responds to nurses, physicians, pharmacists, or other requests to manage, triage appropriately, and ensure follow-up promptly regarding med history information.
3. Obtains patient's allergies & intolerances and communicates to appropriate caregiver for proper documentation into the electronic medical record (EMR), including symptoms, reactions, and severity.
4. Directs, triages, and communicates to appropriate caregiver all questions and inquiries from patient/family/care partner outside the scope of the MHT.
5. Identifies and participates in ongoing quality assurance and improvement processes to obtain an appropriate med history list.
6. Fulfills all compliance responsibilities related to the position.
7. Maintains and Models Nuvance Health Values.
8. Demonstrates regular, reliable and predictable attendance.
9. Performs other duties as required
