Job summary

Talented Medical Solutions is seeking a hospital pharmacy professional responsible for medication reconciliation and pharmacy operations across the patient journey from admission to discharge. You will ensure accurate medication information and collaborate with providers to resolve discrepancies.

The role requires Epic experience and at least 3 years in hospital pharmacy, with IV experience preferred. The department handles inpatient drug distribution and pharmaceutical consultation in multiple

Responsibilities

• Reconcile home medications with inpatient medication orders during hospital stay.
• Assist with medication reconciliation for high‑risk patients at discharge.
• Identify and communicate medication discrepancies to providers.
• Collect and analyze medication reconciliation data.
• Support pharmacy staff under the direction of pharmacy leadership.
• Explain and support drug distribution and consultation across inpatient and ambulatory areas, excluding retail settings.

Job Summary:

Supports medication reconciliation and pharmacy operations, helping ensure accurate medication information throughout the patient's hospital stay and at discharge.

Key Responsibilities:

Reconcile home medications with inpatient medication orders.

Assist with medication reconciliation for high-risk patients at discharge.

Identify and communicate medication discrepancies to providers.

Collect and analyze medication reconciliation data.

Support pharmacy staff under the direction of pharmacy leadership.

This department (which does NOT include administration) controls, mixes, compounds, and distributes drugs, and provides pharmaceutical consultation in support of inpatients, as well as to ambulatory areas including ambulatory/outpatient clinics, infusion therapy, infusion oncology, ambulatory dialysis, ambulatory surgery, ambulatory procedural areas, Operating Rooms and the Emergency Department. Prescription-based Retail entities are excluded.
