Qualifications

• High school diploma required; associate degree preferred.
• Three to five years of pharmacy technician experience preferred.
• Massachusetts Board of Registration in Pharmacy certification/registration required or eligibility as Pharmacy Intern.

Responsibilities

• Triages patients for intake medication histories.
• Conducts patient interviews to obtain complete histories.
• Documents histories into the electronic medical record (EPIC).
• Ensures source verification information is available for prescribers.
• Completes comprehensive pharmacy consult notes in EPIC.
• Escalates issues to ED Pharmacist or management team when needed.
• Updates patient pharmacy information in the Ambulatory Order Management System.
• Collaborates with practitioners to complete tasks and ensure accurate reconciliations.
