Responsibilities

• Assist pharmacist to obtain an accurate medication history and communicate with prescribers.
• Create an electronic medication history note in the patient record.
• Identify errors or discrepancies and communicate with the pharmacist and medical team.
• Adhere to infection control and safety standards; punctual attendance and overtime as needed.
