Job summary

Qualifications

• Minimum 2 years of pharmacy experience.

Responsibilities

• Dispense medications per physician orders and hospital policy.

Description:The Pharmacist is responsible for providing safe, accurate, and effective medication management for patients within the hospital setting.

This role works collaboratively with physicians, nurses, pharmacy technicians, and other members of the interdisciplinary care team to optimize medication therapy and promote positive patient outcomes.

Responsibilities include reviewing and verifying medication orders for accuracy, appropriate dosing, allergies, interactions, contraindications, and therapeutic effectiveness.

Dispensing medications in accordance with physician orders, hospital policies, and state and federal regulations is required.

The Pharmacist will provide clinical recommendations regarding medication selection, dosing, monitoring, and adjustments.

Monitoring patient medication therapies, laboratory results, renal/hepatic function, and other relevant clinical information is necessary.

Participation in medication reconciliation and support for safe transitions of care is expected.

The role includes preparing, verifying, and overseeing sterile and non-sterile medication compounding as applicable.

Oversight and guidance to pharmacy technicians and support staff is part of the responsibilities.

Responding to medication-related questions from physicians, nurses, patients, and other healthcare professionals is required.

Participation in emergency response situations, including code blue and other critical patient events, is necessary.

Maintaining accurate medication records and documentation within the electronic medical record is essential.

Ensuring compliance with controlled substance regulations, medication storage requirements, and hospital pharmacy policies is a must.
