Baptist Health Lexington is seeking a Pharmacy Medication Reconciliation Technician for a full-time second shift to support the hospital's pharmacy services. Responsibilities

• Obtain medication and allergy histories from patients, family, pharmacies, and facilities to support hospital admission and inpatient care.
• Communicate medication history findings to pharmacists and clinicians to determine treatment plans.
• Create and maintain an accurate medication reconciliation list with drug names, doses, frequencies, routes.
• Document changes to patient medication lists in the EMR.

These services include, but are not limited to verification of medication orders, drug distribution, sterile product preparation, hazardous drug preparation, and utilization of automated dispensing cabinets.**Description:*** Supports pharmacy services by obtaining medication and allergy histories for patients that will be admitted to the hospital.
