Job summary

Baptist Health Lexington is seeking a Pharmacy Medication Reconciliation Technician for full-time second shift in the hospital setting. The role involves gathering histories, ensuring accurate reconciliation lists, and communicating changes to pharmacists to support safe patient care.

Responsibilities include interviewing patients and families, reviewing medications, and documenting updates in the EMR to support hospital medication safety and optimal therapy outcomes.

Qualifications

• 6 months of pharmacy experience required.
• Nationally Certified as pharmacy technician.
• Required Kentucky Board of Pharmacy registration certificate for Pharmacy Technician.
• Less than High School.

Responsibilities

• Obtains medication and allergy histories by interviewing patients, families, and external sources.
• Communicates information to pharmacists and providers to determine treatment plans and orders.
• Obtains accurate medication and allergy history from patients, families, pharmacies and long-term care facilities.
• Screens patient medications for duplications, contraindications, or other issues and informs the pharmacist.
• Creates a medication reconciliation list with drug name, dose, strength, frequency, route, and how the patient takes medications.
• Documents changes in the historical medication list within the EMR.
