Qualifications

• Unrestricted Alaska Pharmacist license required.
• Active Basic Life Support (BLS) certification.
• Experience in inpatient hospital pharmacy is preferred.

Responsibilities

• Review prescriptions for therapy appropriateness, safety and interactions.
• Provide clinical consultation with prescribers to resolve issues prior to dispensing.
• Document all clinical activities and interventions accurately.
• Enter prescriptions into pharmacy software and verify orders in a timely manner.
• Manage daily activities of pharmacy staff and supervise technicians.

Astrya Global, a San Diego based medical staffing agency, is hiring a PHARM for a temporary assignment in Sitka, Alaska. These positions are 13-week contracts with the ability to extend.

Responsibilities for a PHARM

• Review all prescriptions for appropriateness of therapy, legality, completeness, contraindications, interactions, allergies, adverse drug reactions, and lab value abnormalities, including screening patient charts
• Provide clinical consultation and clarification, resolving concerns or questions with prescribers prior to dispensing
• Document all clinical activities and interventions accurately and completely
• Enter prescriptions into pharmacy software and verify finished prescription orders in a timely and accurate manner
• Maintain all required record keeping and documentation while working independently with minimal supervision
• Organize and prioritize work assignments to ensure pharmacy services are provided in a timely manner
• Advise patients clearly and accurately regarding their medications, appropriate outcomes, drug name and strength, usage instructions, and expected changes
• Communicate regularly with patients and healthcare providers within and outside the facility to resolve problems and provide optimal care
• Perform chart reviews for prescription refills and medication therapy management
• Write recommendations for providers and request prescription refills on patients' behalf
• Provide drug information, drug therapy consultation, and staff education relating to drug therapy
• Direct and verify the daily activities of pharmacy support personnel, serving as a role model and educator for pharmacy students and technicians
• Participate in clinical and administrative activities including quality control and quality improvement within the department and clinic
• Attend meetings and participate on committees as requested
• Assure availability, preparation, and control of medications and select appropriate drugs for formulary
• Work in pharmacy-run clinics and take on additional departmental duties and responsibilities as assigned
• Provide health promotion and disease prevention activities related to drug use and preventative drug therapy
• Maintain current Basic Life Support (BLS) certification for Healthcare Providers
• Maintain an unrestricted license as a registered pharmacist in the State of Alaska
• Maintain current flu vaccination
