Job summary

Position Summary

This is a FT hybrid position based in Portland. This role collaborates with the Director of Population Health, Health Equity and Quality, plan staff, communities, healthcare providers, pharmacies, prescribers, and other partners to advance health equity. The position focuses on pharmacy-related health equity and quality improvement, facilitating culture change and addressing patient experience, outcomes, access to care, health related barriers and social needs. It also supports efforts to reduce health inequities caused by factors such as racism, poverty, ineffective policies, and systemic obstacles.

Pay Range

$65,682.16 - $82,107.97 annually (depending on experience). This role may be classified as hourly (non-exempt) depending on the applicant's location. *Actual pay is based on qualifications. Applicants who do not exceed the minimum qualifications will only be eligible for the low end of the pay range.

Required Skills, Experience & Education:
  1. Bachelor's degree in Public Health or an equivalent combination of education and relevant work experience.
  2. 2 years of relevant experience including working in or with rural or underserved communities, working with leadership, facilitating meetings, measuring, and monitoring programs evaluation activities and working with multi‑disciplinary, multi‑cultural teams.
  3. Equity advocacy or system change efforts experience preferred.
  4. Document and report quality initiatives to appropriate internal and external stakeholders.
  5. Through healthcare research and quantitative analysis, summarize and synthesize complex data and information to identify trends and patterns, pinpoint issues, and areas of improvement, and recommend solutions and next steps.
  6. Coordinate with marketing staff and vendors as needed to update, create, or purchase relevant materials and communications.
  7. Work with other departments to develop and vet program materials using guidelines established by state partners and the department.
  8. Ensure compliance with applicable state and national quality assurance and quality improvement standards.
  9. Develop annual project work plan resource allocations and budget impacts.
  10. Oversee data collection and analysis of data and prepares pertinent documentation and reporting for quality improvement activities, including queries and member and provider surveys.
  11. Monitor and report the outcome of health promotion and quality improvement committees, regulatory agencies, and other oversight entities.
  12. Develop and maintain quality improvement policies and processes.
  13. Review pharmacy policies and procedures on an annual basis to address health inequities.
  14. Assist with pharmacy health promotion activities to improve the quality and safety of clinical care programs/services provided to pharmacy members.
  15. Plan and assign quality activities and tasks with a variety of outside entities and a diverse work force, including leadership, operational staff, regulatory, business analysts, technical teams, marketing, and finance.
  16. Seek out and participate in professional growth and development activities.
  17. Other duties and assigned.
Primary Functions:
  1. Plan and execute productive convenings with constituents (i.e., Community Advisory Council) to identify and advance shared goals, clarify challenges, develop strategies to build solutions, and assure progress through consensus agreements and actionable next steps.
  2. Develop focused interventions to address social determinants of health, implicit bias, institutional racism, systemic barriers to race and health equity and strategies to shift practice in clinical settings.
  3. Participate in the Medical Quality Improvement Committee (MQIC).
  4. Provide leadership and support the review and coordination of communication materials to ensure alignment with Culturally and Linguistically Appropriate Services (CLAS) and Health Equity standards and initiatives.
  5. Oversee the administrative functions related to Community Advisory Council Operations.
  6. Organize and Conduct Quarterly Community Advisory Committee meetings with key constituents to build relationships, share best practices and health equity tactics, develop a health equity plan, and provide quality resources or services that add value to the members.
  7. Collaborate with, leverage the skills of, influence and motivate partners and stakeholders across the service area, by providing education and change management strategies as needed.
  8. Represent Moda at meetings and conferences.
  9. Within Pharmacy Health Equity Quality Improvement, work on meeting the Quality Metrics and implement interventions, as defined.
  10. Provide oversight and coordination of pharmacy related to health equity regulatory contracts and accreditation.
Working Conditions & Contact with Others:
  • Office environment with extensive close PC and keyboard work, constant sitting, and phone work. Must be able to navigate multiple screens. Must be able to travel within the service delivery area and some out of service area travel, including overnight on occasion. Work in excess of 40 hours per week, including evenings and occasional weekends, to meet business need.
  • Works with multiple internal departments and externally with the community, public health, hospitals, providers, pharmacies, physical and behavioral health providers, UMP, Oregon Health Authority and Washington Healthcare Authority stakeholders.
