Job Summary
This position is 9:30 p – 7:30 a, Wednesday through Tuesday, 7 on 7 off.
Provide pharmaceutical care to patients with an emphasis on clinical monitoring and therapeutic interventions that improve patient outcomes. Provide information and educational programs to customers. Assist with the maintenance of performance improvement processes to constantly improve our medication use process. Assure dispensing accuracy and direct pharmacy workflow to ensure accurate and timely dispensing and distribution.

Job Duties
- Medication Order Verification: review and interpret medication orders, ensuring completeness and appropriate dosing. Assess drug interactions and allergies. Obtain clarification when needed and provide therapeutic alternatives.
- Therapeutic Clinical Monitoring: participate in ongoing monitoring of medication therapy, direct therapy within collaborative care agreements, contact prescribers as needed, document assessments and recommendations.
- Compounds and Dispenses Pharmaceuticals: supervise compounding, ensure correct labeling and dosage, use barcode technology where possible, supervise repackaging of medication.
- Supervise and Direct Interns and Technicians: provide active supervision and mentoring, monitor task completion, use AIDET model in interactions, report non-responsive behavior to leadership.
- Medication Use Safety Initiatives: detect and report variances in dispensing, identify opportunities for improvement, report suspected adverse drug reactions.
- Controlled Substance Accountability: supervise receipt, dispensing, and distribution of controlled substances, maintain accurate inventories, reconcile reports.
- Financial Contribution: encourage prescriber compliance with formulary restrictions, process non-formulary requests, participate in IV to PO conversions, manage time efficiently.
- Drug Information / Staff Development: provide accurate drug information, serve as preceptor, develop staff education programs.
- Professional Competency: maintain licensure, complete competency assessments, maintain current knowledge of pharmacy practice.
- Perform other duties as assigned.
Qualifications
Education, Credentials, Licenses
- Graduate from an ACPE accredited pharmacy college
- Current U.S. pharmacist license in the state where work is performed
Specialized Knowledge
- Problem solving and critical thinking skills
- Good communication skills
- Familiar with current practice standards (ASHP)
