Provide Pharmaceutical Care to patients with an emphasis on clinical monitoring and therapeutic interventions that improve patient outcomes. Provide information and educational programs to customers and assist with the maintenance of Performance Improvement processes to continually improve our Medication Use Process. Ensure dispensing accuracy and direct pharmacy workflow to assure accurate and timely dispensing and distribution.

Medication Order Verification: Review and interpret medication orders from licensed prescribers or their agents, ensuring they meet the requirements of a complete medication order. Verify that the medication and dose are appropriate, assess drug interactions, allergy alerts, duplication of therapy, and potential therapeutic interactions. Obtain clarification from the prescriber for any incomplete or suboptimal orders and provide therapeutic alternatives. Perform a second verification of medication orders for pediatric patients and chemotherapy orders.
Therapeutic Clinical Monitoring: Actively participate in ongoing monitoring of medication therapy in compliance with approved Collaborative Care Agreements and medication management protocols. Direct medication therapy within the bounds defined by agreements and protocols. Contact prescribers as needed with therapeutic alternatives. Document therapy assessments and recommendations in the patient’s chart using iVents and progress notes, keeping the interdisciplinary team informed.
Compounds and Dispenses Pharmaceuticals: Ensure timely dispensing of appropriate quantities of the correct medication. Supervise compounding according to USP standards, verifying ingredient compatibility, content, labeling, and expiration dates. Check all medication prior to final dispensing for direct patient administration, floor stock, or automated dispensing cabinets. Use barcode technology to ensure accurate dispensing. Supervise repackaging, including verification and documentation of lot numbers, expiration dates, and personnel names.
Supervises and Directs Interns and Technicians: Provide direct supervision, mentoring, and task monitoring per ICARE values. Encourage use of the AIDET model and report nonresponsive behaviors to the leadership team.
Medication Use Safety Initiatives: Detect and report variances in dispensing, identify opportunities to improve medication safety, and report suspected adverse drug reactions.
Controlled Substance Accountability: Participate in and supervise the receipt, dispensing, and distribution of controlled substances through Pyxis and other systems, reconcile inventories, and assist in resolving discrepancies.
Financial and Therapeutic Interventions: Encourage prescriber compliance with formulary restrictions, process non-formulary requests, suggest alternatives, participate in IV to PO conversions, and minimize overtime by efficient time management.
Drug Information / Staff Development: Provide accurate drug information and serve as a preceptor for residents, interns, and pharmacists. Develop staff education programs.
Maintains Professional Competency: Maintain licensure, complete competency assessments, stay current in pharmacy practice, and develop specialized skills.
