Job summary

Responsibilities

  • Detect and generate alternatives for problematic medication orders.
  • Train pharmacists and technicians on IV preparations and unit dose.
  • Process chemotherapy orders and ensure correct dosages are checked.
  • Serve as a resource for medication queries and recommendations.
  • Function in any central pharmacy area needing an R.P.H.

Job description

Job Responsibilities
  • Evaluate medication orders based on patient weight, allergy history, and compatibilities; record orders in the patient profile; complete orders without error and ready for delivery with the next run; clarify and resolve problem orders; contact physicians or nurses as needed.
  • Detect all problematic medication orders, generate appropriate alternatives, and effect changes in patient care.
  • Train other pharmacists and technicians in unit dose and IV areas; prepare all IV preparations (TPN, central lines, antibiotics, chemotherapy, respiratory therapy syringes) following departmental guidelines; check for errors and ensure delivery per policy.
  • Process chemotherapy orders; ensure dosages and calculations are checked by two pharmacists; use precautions when preparing chemotherapy (gowns, gloves, chemotherapy mat), prepare in appropriate hood, and use calculations on the patient cardex.
  • Serve as a resource by using appropriate references to ascertain correct medication answers; review patient data at physician request to recommend time for serum concentrations; handle questions and problems professionally—especially on the telephone and at the window; anticipate conflict, take preventative or corrective action; identify concerns and assist in problem solving and decision making.
  • Function in any central pharmacy area where an R.P.H. is needed; take administrative initiative when supervisor is not available; serve as an informational resource for the institution.
