Job summary

Memorial Physician Practices is looking for a skilled Pharmacist in Fort Worth, Texas, to manage drug therapy protocols as a vital part of our interdisciplinary team. Your role will involve reviewing medication orders, providing drug information, and documenting interventions to enhance patient care.

We require applicants to have at least a Bachelor’s Degree in Pharmacy and a valid Pharmacy License in Texas. Candidates must possess critical thinking skills, the ability to work with minimal supervision, and CPR certification within 30 days of hire.

Qualifications

• Pharmacy License in the state of Texas.
• CPR and de-escalation certification required within 30 days of hire.

How you'll contribute

A Pharmacist who excels in this role:

• Manages drug therapy protocols as a part of an interdisciplinary team.


• Reviews, interprets, and evaluates medication orders to identify, prevent and resolve potential or actual drug-related problems.


• Provides drug information and education to medical staff, healthcare professionals, and patients, and pharmacy students.


• Documents interventions and their outcomes for use in drug therapy quality improvement efforts.


• Evaluates medication orders for accuracy and completeness prior to order entry.


• Checks for accuracy after filling by technicians.

What we're looking for

Applicants should have a Bachelor's Degree (required); Doctorate Degree preferred. Additional requirements include:

• Pharmacy License in the state of Texas.


• Requires critical thinking skills, decisive judgement, and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.


• CPR and de-escalation certification required within 30 days of hire and prior to any patient contact.
