Job summary

Compassus is seeking a Pharmacist to support infusion therapy patients by ensuring safe and effective medication use. You will work under a Pharmacy Manager to interpret orders, prepare IV admixtures, dispense medications, and monitor therapies in collaboration with nursing and medical staff.

Responsibilities include clinical reviews, documentation, inventory management, and staff supervision. A PharmD or Bachelor of Pharmacy and licensure are required, with experience in IV care preferred.

S/he works under the guidance of the Pharmacy Manager and/or Pharmacy Supervisor to fulfill all necessary duties.Position Specific ResponsibilitiesInterprets and evaluates all physician’s medication orders to assure therapeutic appropriateness, cost-effectiveness, accuracy, and completeness.Participates with nurses and physicians in all clinical activities required, including the start of care, prospective review of medical and drug histories, pharmacokinetic dosing, dosage monitoring for compromised patients, reviewing laboratory test results, attending patient care conferences, developing and updating patient-specific plans of care, and patient discharge consultation.Provides technical and clinical information and communicates results of monitoring activities to patients, nurses, and physicians regarding the contents, utilization, therapeutic actions, and possible adverse reactions or interactions of drugs.Performs accurate computer order entry, labeling, and dispensing.Compounds IV admixtures accurately, utilizing the appropriate technique, performing checks, and double checks per policy and procedure.Accurately maintains all legally required records.Supervises pharmacy support team members and/or interns in the appropriate mixing, packaging, and labeling of pharmaceuticals.Assists in training and orienting new team members.Initiates, reviews, and updates all required documentation per policy and procedure.Follows established procedures for drug and supply procurement and maintains appropriate inventory levels and preferred product selection of drugs, solutions, and supplies.Adheres to stability and compatibility guidelines and quality control procedures.Maintains procedures for appropriate security of all legend items, including controlled substances.Aids in the referral acceptance/denial process and product device supply selection.Participates in on-call rotations.Engages in Performance Improvement activities, educational programs, and departmental/branch meetings.Drives the Primary Care Management Model in daily interactions through the delegation of non-cognitive activities such as (but not limited to) lab tracking to support personnel, ordering clinically necessary and cost-effective labs, and facilitation of positive patient outcomes with patients, nursing agencies, nursing, and other healthcare providers.Performs other duties as assigned.Education and/or ExperienceBachelor's degree in Pharmacy or a PharmD required.Minimum of one (1) year experience in an IV alternate site environment or equivalent work experience preferred.SkillsMathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
