Job summary

JobRx, Inc. seeks a licensed pharmacist for per diem daytime coverage in Texas to provide pharmaceutical care, oversee dispensing, and collaborate with physicians to optimize medication use.

Responsibilities include reviewing therapies, verifying orders, counseling staff and patients, ensuring regulatory compliance, and participating in education and quality assurance within a hospital setting.

Qualifications

  • Pharmacy license is required.
  • ACLS, BLS, PALS required if direct patient care responsibilities.
  • D. or BS in Pharmacy (Pharm.) is required.

Responsibilities

  • Review and monitor patient medication therapy and orders for safety and efficacy.
  • Advise patient care team on medication therapy and document interventions.
  • Order verification and medication profile reviews for duplications, interactions, allergies, dosing.
  • Prepare, label, and dispense medications per prescriptions; ensure compliance with laws.
  • Check High Risk IVs, renal dosing, IV to PO conversions, modify therapy, and manage box warnings.
  • Ensure medications are available, procured, stored, and maintained.
  • Supervise the preparation, maintenance, and dispensing processes.
  • Maintain relationships with medical staff; provide drug information and regulatory updates.
  • Provide collaborative practice management for anticoagulation, parenteral nutrition, vancomycin, etc.
  • Assist with medication reconciliation and clarification of unsafe orders.
  • Provide education to patients on medications.
  • Participate in education and training of healthcare professionals.
  • Demonstrate regulatory compliance via inspections.
  • Maintain professional competence via staff development and study.

Per Diem, Days

Provide pharmaceutical care services to the patients including, but not limited to oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy as well as providing drug information, patient information and oversight of information and dispensing systems. Additionally: ensures compliance with Federal, State, and local laws and regulations. May also advise physicians on medication utilization.

Responsibilities
  • Review and monitor patient's medication therapy and orders written for safety and efficacy considering specific patient conditions and health status
  • Advise patient care team on patient's medication therapy and document such interventions
  • Order verification and performing medication profile reviews that should include but not limited to checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency
  • Prepare, label, and dispense medications for patients in accordance with physicians' prescriptions; ensure compliance with Federal, State, and local laws and regulations pertaining to the dispensing of drugs and narcotics and the maintenance of required records
  • Check High Risk IVs, renal dose adjusting medications, IV to PO conversions, initiates/modifies/ discontinues therapy, and mediates black box warning interventions
  • Ensure that appropriate medications are available for patients and properly procured, prepared, stored, and maintained
  • Supervise the preparation, maintenance, and dispensing process of drugs and supplies, including technical personnel performing these functions
  • Maintain relationships with medical and nursing staff; provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission standards
  • Provide for collaborative practice management as per facility-approved protocols for anticoagulation, parenteral nutrition, vancomycin, and aminoglycoside consults, etc.
  • Assist with medication reconciliation and clarification of unsafe and unclear drug therapy orders
  • Provide education to patients on the use of medications as needed
  • Participate in the education and training of healthcare professionals (e.g. physicians, nurses, pharmacy technicians, and students) in relation to medication use
  • Demonstrate regulatory compliance as evidenced by timely completion of nursing unit inspections, as needed
  • Maintain professional competence with pharmacy department and state licensing requirements via staff development, independent study, and/or professional activities
Qualifications
Education
  • Required: Pharm.
  • D. or BS in Pharmacy
Experience
  • Preferred: Hospital experience
Certifications
  • Required: Pharmacy license. AHA BLS, ACLS, PALS required if direct patient care responsibilities.
  • Preferred: Board Certification in Pharmacotherapy (BCPS), CGP, etc.
Physical Demands

Lift/position up to 50 lbs. Push/pull up to 50 lbs. of force. Frequent standing and walking. Frequent reaching, stooping, and bending. Manual dexterity, mobility, touch, auditory to perform all the related duties of the position.
