Qualifications

Job description

SUMMARY:

The PRN Registered Pharmacist provides comprehensive pharmacy services within a hospital setting, ensuring safe, effective, and evidence-based medication use across inpatient, outpatient, and emergency settings. This role works closely with physicians, nurses, and other clinical staff to support optimal patient outcomes in a rural community hospital. The pharmacist is responsible for clinical review, medication verification, compounding, regulatory compliance, and direct patient education.

KEY RESPONSIBILITIES:
  • Medication Management & Clinical Review
    • Verify medication orders for accuracy, safety, and appropriateness.
    • Provide clinical pharmacy services, including dose adjustments, renal dosing, antimicrobial stewardship support, and therapeutic monitoring.
    • Review medication profiles and collaborate with providers to optimize therapy.
    • Counsel patients on medication use, side effects, and adherence.
  • Dispensing & Compounding
    • Prepare and dispense medications in accordance with state and federal regulations.
    • Compound sterile and non-sterile products following USP <797>/<800> standards.
    • Oversee and support pharmacy technicians in daily operations.
    • Ensure timely delivery of medications to inpatient units and the emergency department.
  • Regulatory, Safety, & Quality Compliance
    • Maintain compliance with Illinois Board of Pharmacy regulations, CMS requirements, and hospital policies.
    • Manage controlled substances, including inventory, documentation, and audits.
    • Participate in medication safety initiatives, quality improvement projects, and internal reviews.
    • Maintain accurate and complete documentation in the electronic health record.
  • Interdisciplinary Collaboration
    • Serve as a medication expert for physicians, nurses, and other clinical staff.
    • Participate in patient care rounds when applicable.
    • Provide drug information, education, and support for clinical decision-
    • Assist with transitions of care, including discharge medication counseling.
  • Operational Support in a Critical Access Setting
    • Support pharmacy operations across all hospital departments due to the small-team environment.
    • Assist with formulary management, medication procurement, and shortage mitigation.
    • Participate in weekend and holiday on-call rotations.
    • Help maintain a clean, organized, and compliant pharmacy environment.
  • Other duties as assigned.
QUALIFICATIONS AND REQUIREMENTS:
  • Active Illinois Registered Pharmacist (RPh or PharmD) license in good standing.
  • Strong knowledge of hospital pharmacy practice, medication safety, and clinical therapeutics.
  • Excellent communication, problem-solving, and critical-thinking skills.
  • Ability to work independently and collaboratively in a small, fast-paced environment.
  • Prior experience in hospital or clinical pharmacy.
  • Experience with sterile compounding and hazardous drug handling.
  • Familiarity with electronic health records and automated dispensing systems.
  • Background in antimicrobial stewardship, emergency medicine, or rural healthcare.
