Job summary

Dignity Health East Valley Rehabilitation Hospital – Chandler is seeking a Pharmacist to join our inpatient rehabilitation team. The role focuses on accurate medication orders, medication management processes, and collaboration with clinicians to ensure safe, evidence-based therapy.

The ideal candidate holds a BS Pharm or PharmD, an active state license, and hospital pharmacy experience. PRN position with involvement in anticoagulant monitoring and compliance initiatives.

Qualifications

  • Graduate of an accredited school of pharmacy; BS Pharm or PharmD required.
  • Accredited clinical residency preferred.
  • Current state license to practice pharmacy.
  • Two years hospital pharmacy experience preferred.

Responsibilities

  • Verify physician medication orders with accuracy and timeliness.
  • Monitor and evaluate medication management processes to drive quality improvement.
  • Verify and approve work by Pharmacy Technicians; provide coaching.
  • Order, receive, and store medications maintaining integrity and compliance.
  • Review patient-specific drug therapy for indications, dosing, interactions, and contraindications.
  • Perform anticoagulant monitoring and dose recommendations as per protocol.
  • Ensure compliance with hospital policies and regulations; maintain records.

Dignity Health East Valley Rehabilitation Hospital - Chandler rehabilitation programs provide ongoing care and specialized treatment to patients throughout their recovery journey. We offer customized, individualized rehabilitation tailored to the specific needs of those recovering from a stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury.

How you’ll contribute

A Pharmacist who excels in this role:

  • Verify physician medication orders with accuracy and timeliness; clarify and resolve discrepancies.
  • Monitor and evaluate the critical processes of medication management (e.g., selection, storage, ordering/transcribing, preparation/dispensing, administration support, and monitoring); identify risk points and drive continuous quality and performance improvement with clinicians.
  • Verify and approve work performed by Pharmacy Technician(s); provide coaching and feedback.
  • Order, receive, and store medications to maintain integrity, security, and compliance.
  • Review patient-specific drug therapy for indications, dosing, interactions, contraindications, and duplication; promote rational, evidence-based therapy and document interventions.
  • Perform anticoagulant monitoring and communicate dose/therapy recommendations per protocol or provider order.
  • Ensures compliance with hospital policies, state/federal regulations, and accreditation standards. Maintains accurate records (e.g., inventory, controlled substances, medication safety reports). Supports medication safety initiatives, adverse event prevention, and staff education. Utilizes pharmacy information systems and automated dispensing technologies as required.
  • Perform other duties as assigned.

What we’re looking for
  • Graduate of an accredited school of pharmacy; BS Pharm or PharmD required; accredited clinical residency preferred.
