Responsibilities

• Ensure safe transitions of care via comprehensive medication reconciliation and final verification on admission and discharge orders.
• Evaluate physician orders for appropriateness, dosing, interactions, and safety.
• Clarify orders with prescribers when needed to ensure accurate medication therapy.
• Support antimicrobial stewardship and optimize therapy through evidence-based regimens.
• Assist with monitoring and appropriate prescribing of anticoagulation therapy.
• Provide drug information and pharmacotherapy recommendations to clinical staff.
• Supervise pharmacy technicians in dispensing, ordering, and inventory activities.
• Document all clinical interventions thoroughly and accurately.

Qualifications

Required Skills:

Responsibilities

• Ensures safe transitions of care by completing comprehensive medication reconciliation and final verification of admission and discharge orders for all patients.
• Evaluate and verify WellSky physician's orders for therapeutic appropriateness, proper dosing, optimal dosage form, drug interactions, allergic reactions, route and rate of administration and therapeutic redundancy
• Clarifies physicians' orders, when necessary, through direct and timely contact with prescriber or designated practitioner
• Demonstrates knowledge with antimicrobial stewardship , which includes therapy optimization, de-escalation, and selection of appropriate, evidence-based antimicrobial regimens.
• Assists with monitoring and appropriate prescribing of anticoagulation therapy
• Provides patient-specific pharmacotherapy recommendations and drug information to the medical and nursing staff
• Supervises pharmacy technicians in activities related to medication dispensing, ordering and inventory management.
• Thorough documentation of all clinical interventions
