Job summary

UNC Rex Healthcare’s Pharmacy Team is seeking a per diem Clinical Pharmacist to provide patient care within decentralized clinical services at Rex IP Pharmacy in Raleigh, NC. You will verify orders, review patient data, and support inpatient teams across cardiology, internal medicine, neurology, and surgical services.

Responsibilities include antimicrobial management, medication counseling, and responding to drug information requests while adhering to all policies.

Qualifications

• PharmD or BS Pharmacy Degree is required.
• Active pharmacist licensure in NC or able to obtain before start date.
• Bachelor’s or Doctor of Pharmacy degree currently held or in progress.

Responsibilities

• Review and interpret orders to assure accuracy and appropriateness.
• Provide antimicrobial management and drug information.
• Counsel patients and families on pharmaceutical care issues and lifestyle modifications.
• Participate in Code Blue and adverse event reporting.
• Support inpatient nursing units with pharmaceutical care interventions.

Clinical Pharmacists Work In a Multidisciplinary Environment Alongside Our Inpatient Care Teams. These Positions Will Provide Order Verification Services, Medication Profile Reviews, Response To Drug Information Questions, And Problem Resolution As Needed In The Following Areas

• Cardiology

Responsibilities

• Review and interpret orders to assure accuracy and appropriateness
• Assess relevant patient data, in conjunction with prescribed therapies, and make appropriate pharmaceutical care interventions
• Promote and demonstrate rational, cost-effective drug therapy
• Provide antimicrobial management
• Provide pharmacist support to inpatient nursing units
• Counsel patients and appropriate family members regarding pharmaceutical care issues and lifestyle modifications
• Adverse Drug Event and Medication Error Reporting
• Code Blue participation
• Response to drug information requests
• Adhere to federal, state, local and institutional policies, procedures, standards and regulations

Licensure/Certification Requirements

Active pharmacist licensure with the NC Board of Pharmacy or obtainable before initiation of employment. Recent graduates of ACPE-accredited colleges/schools of pharmacy may fill this role while pending initial pharmacist licensure, but are subject to limitations mandated by the NC Board of Pharmacy and must obtain licensure within 6 months of orientation or sooner if required for operations.
