Job summary

TriHealth in Norwood, Ohio is seeking a Pharmacist for the PACE of Cincinnati program and Hospice of Cincinnati. This full-time role supports PACE clinical and dispensing functions, with schedule M-F 11:30 AM–8:00 PM, serving participants across community and care settings.

The PACE Clinical Pharmacist conducts initial medication assessments and reassessments as part of the IDT care planning, ensures safe drug therapy monitoring, and provides education to participants, caregivers, and staff.

Qualifications

  • Bachelor's degree or Pharm D with current Ohio license.
  • 1–2 years of experience with frail, elderly, or long-term care populations.
  • Basic Life Support (BLS) certification required.

Responsibilities

  • Completes comprehensive initial medication management assessments for new PACE participants and reassessments for existing participants as part of the IDT care plan.
  • Develops participant care plans for medication management in collaboration with the IDT and primary care providers.
  • Acts as liaison between contracted pharmacy provider and PACE staff to ensure safe delivery and distribution of medications.

TriHealth is actively recruiting for a Pharmacist to join the PACE of Cincinnati PACE Center located in Norwood Ohio. Program of All-Inclusive Care for the Elderly (PACE) is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility.

This unique opportunity is for a full-time position, M-F 11:30 AM- 8:00 PM to service in a position to support PACE of Cincinnati and Hospice of Cincinnati as a clinical and dispensing pharmacist. The PACE Clinical Pharmacist is responsible for completing comprehensive initial medication management assessments for new PACE participants and medication management reassessments on existing PACE participants. For Hospice of Cincinnati, as the dispensing pharmacist, drug distribution skills including order entry, clarification if needed, accuracy of checking, technician supervision and timely delivery. Effectively manage day to day operations, handle initial conflict management, provide operational feedback and opportunities, implement corporate and departmental directives, and manage expenses within budget. Oversees and assists in the development of service-oriented staff through managing effective workflow and actively participating in the training of new employees.

Job Overview:

The PACE Clinical Pharmacist is a member of the PACE Interdisciplinary Team (IDT) and is responsible for completing comprehensive initial medication management assessments for new PACE participants and medication management reassessments on existing PACE participants as part of the PACE IDT care planning process. In compliance with state and federal regulations, the PACE Clinical Pharmacist assures safe and effective medication management protocols by providing industry best-practice drug therapy monitoring regimens and ongoing medication management education to participants and caregivers. For Hospice of Cincinnati, responsible for the filling and dispensing of medication orders including the proper handling, usage, and storage of drugs in the pharmacy. Maintains standards set by the department and translates policies and procedures into the daily operation of the pharmacy. Responsible for acting as an information resource to patients, physicians, nurses, and other healthcare professionals regarding medication therapy and drug related information and literature. Plans and organizes work schedule of supportive personnel and pharmacists, as required.

Job Responsibilities:
  • Participates as a PACE Interdisciplinary Team (IDT) member and is responsible for completing a comprehensive initial medication management assessment for new PACE participants and medication management reassessments on existing PACE participants as part of the PACE IDT care planning process.
  • In collaboration with the IDT and primary care providers, develop participant care plans for medication management considering the medical, physical, social, and emotional needs of PACE participants.
  • Acts as a liaison between the contracted pharmacy provider and PACE provider staff to ensure the safe and effective delivery and distribution of medications to PACE participants.
  • In compliance with state and federal regulations, the PACE Clinical Pharmacist assures safe and effective medication management protocols by providing industry best-practice drug therapy monitoring regimens and ongoing medication management education to participants and caregivers.
  • In collaboration with the IDT, responsible for the medication management of PACE participants across the care continuum, including community settings, hospitals, and skilled nursing facilities. Provides medication management support and guidance during care transitions from the hospital and skilled nursing facilities back to community settings. Completes and maintains medication management documentation in the PACE EHR in accordance with the federal PACE regulation and organizational policy. Provides medication management education to participants, caregivers, and PACE staff.
  • Attends IDT and morning meetings and reports on participant's medication management status. Participates in the Fraud, Waste, and Abuse Committee. Supports the PACE QI program and participants in PACE QI activities as assigned. For Hospice of Cincinnati, assumes personal responsibility for professional development, particularly in the area of ongoing clinical education. Demonstrates a working knowledge of computer operations, all third party plans, ensuring pharmacy support staff is knowledgeable in the same areas, trouble shooting, and call resolution. Demonstrates initiative in accepting additional projects. Seeks areas of improvement. Drug distribution skills including order entry, clarification if needed, accuracy of checking, technician supervision and timely delivery.
