Job summary

UVM Health is seeking a Pharmacist for outpatient services to support ambulatory pharmacy patients across Retail, Mail Order, Home Infusion, and more. You will dispense medications, screen for therapy issues, and counsel patients and healthcare professionals to optimize outcomes.

Work with multidisciplinary teams to resolve medication-related problems, address financial barriers, and uphold sound business practices and clinical judgment in a dynamic ambulatory setting in Burlington, Vermont.

Qualifications

• Doctor of Pharmacy or B.S. in Pharmacy from ACPE-accredited college.
• Licensed pharmacist in Vermont.

Responsibilities

• Dispense medications according to prescriptions and resolve medication-related problems.
• Counsel patients and healthcare professionals on proper use of medications.

Job Summary:

The Pharmacist, Outpatient services the needs of the UVM health system ambulatory pharmacy patients. The pharmacist may work in one or more areas of the ambulatory pharmacy department, including but not limited to: Retail, Mail Order, Home Infusion, Call Center, Specialty Pharmacy, Travel Clinic, and Transitions of Care. The pharmacists will work with multidisciplinary teams to resolve medication related problems. The pharmacists will also dispense medication according to prescription orders while screening and resolving medication related problems, financial barriers, and counseling patients and health care professionals on proper use of medications. The Pharmacist will maintain good business practices and sound clinical judgement to ensure the success of the department and patient outcomes.

Education:

• PharmD or B.S Pharmacy Degree from ACPE accredited college of pharmacy.
• Licensed pharmacist in Vermont.

Experience:

• Minimum one year experience in a retail/mail/call center/specialty/home infusion pharmacy within past 5 years.
• Minimum one year experience in a health system pharmacy preferred.
