Job summary

IHA in Grand Rapids, MI seeks a part-time Home Infusion Pharmacist to manage IV therapies at home and in the infusion suite. This role includes occasional after-hours/on-call time and weekend shifts to maintain software systems and operations without disruption.

The candidate will blend direct patient care with staff training in a specialty area, requiring strong communication and technical skills and a Michigan pharmacist license.

Qualifications

• Five-year bachelor of science degree in pharmacy from an accredited college of pharmacy.
• Active Michigan pharmacist license with controlled substance license; BCLS may be required.
• Experience in home infusion or hospital pharmacy preferred; strong communication and training skills.

Responsibilities

• Reviews, prepares and dispenses medication orders per physician orders according to policies.
• Ensures safe, cost-effective therapies for patients per guidelines and protocols.
• Provides clinical consultation and drug information to clinicians and patients.
• Trains and precepts pharmacy staff and students as assigned.
• Supervises pharmacy technicians and supports daily operations.
• Maintains software systems and supports operations with weekend/on-call coverage.

What You Will Need

Five year bachelor of science degree in pharmacy from an accredited college of pharmacy. Specialist in one or more areas of expertise within the pharmacy such as pharmacy software maintenance, hardware upkeep, training of pharmacy personnel or communication with outside technical departments (TIS).

Credentials/Licensure

Minimum Current registered pharmacist license and controlled substance license issued by the Michigan State Board of Pharmacy. Basic Cardiac Life Support (BCLS) certification may be required (at discretion of manager).

Minimum Ability to communicate effectively. Knowledge of USP 797 and USP 800.

Computer Competency

Familiarity with standard desktop and windows based computer system, including email, e-learning, intranet and computer navigation. Ability to use other software required to perform essential functions.

Physical/Mental

Minimum Repetitive use of hands and fingers (e.g., preparing IV admixtures, use of a computer keyboard). May require lifting and carrying light loads (up to 40 lbs.), including boxes, equipment, unit dose-cassettes, and IV solutions and stooping or kneeling (e.g., to pick up items from the floor, to remove and replace items on lower shelves, and to file documents in lower file drawers). Sitting, walking, or standing for long periods of time (4-8 hours) is often necessary. Must be able to physically operate the equipment for the job.

What You Will Do

• Reviews, prepares and dispenses medication orders per physician order or prescription according to established policies, procedures, and protocols.
• Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures and protocols.
• Contributes to the quality and effective operation of the department.
• Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to physicians, as needed.
• Provides drug information and counseling to patients, families, and other healthcare professionals.
• Participate in the training, education, precepting, and evaluation of pharmacy students and/or residents as assigned.
• Provides work direction to pharmacy support personnel, verifies the daily activities of pharmacy technicians.
• Performs other related duties as assigned.
