About Us: The Inpatient Pharmacy Department at Valley Children’s utilizes evidence-based therapeutic guidelines to provide exceptional pharmaceutical care to our patients. Our highly trained pharmacists and pharmacy technicians work side-by-side with providers and other interprofessional teams to ensure optimal patient outcomes, while utilizing state-of-the-art technology to maximize patient safety. Inpatient Pharmacy provides clinical interdisciplinary pharmacy services in all inpatient areas including Rehab, PICU, NICU, Acute Care, and Oncology. We have numerous pharmacy advance practice protocols including antibiotic monitoring, anticoagulation, IV to oral conversion, and renal dosing. Other clinically supported services include the Emergency Department, Palliative Care, Investigational Drug oversight, and parenteral nutrition (including production). Our team members also support the following specialty clinics: Oncology, Hematology, Rheumatology, Pulmonology/CF, Endocrinology, Neurology, Cardiology and our complex primary care center. Many pharmacists hold collaborative practice agreements to optimize medication therapy management and collaborate to enhance patient care.
Job Summary: The Pharmacist Lead serves as the clinical expert and operational resource for a designated specialty area (Oncology, Med/Surg, NICU, PICU, Parenteral Nutrition). Works directly with Pharmacy Administration to identify and resolve operational and clinical issues within the department and/or specialty area, holding quality and patient safety to the utmost importance. Works directly with the Pharmacy Administration and specialty area leadership in the creation and modification of pharmacy protocols, policies, and procedures, including implementation and ongoing education. Actively participates in evaluation of pharmacy metrics and recommends innovative operational and clinical improvements to ensure practice excellence. The Pharmacist Lead shall provide education, training, and feedback to management regarding staff performance within the specialty area.
Qualifications: Education Doctor of Pharmacy Degree (required) Program ASHP Accredited Residency Program. Pediatrics preferred but not required. (preferred) Licenses and Certifications RPH - Registered Pharmacist - CA-SBP - California State Board of Pharmacy California State Board of Pharmacy (required) BLS - Basic Life Support - AHA - American Heart Association within 30 days of hire or transfer into position (required) NPI - National Provider ID Number - NPI - National Provider ID Number (required) uired under HIPAA Administrative Simplification (required) PALS - Pediatric Advanced Life Support - AHA - American Heart Association within 12 months of hire or transfer into position (required) ACLS - Advanced Cardiac Life Support - AHA - American Heart Association Encouraged for those serving in this position. Work Experience: Minimum one (1) year Hospital experience. Shall be able to perform all duties as a pharmacist. (required)
