Job summary

Lehigh Valley Health Network (LVHN) is seeking a Pharmacist to fill and dispense prescription orders in accordance with PA law and current pharmacy standards. The role involves reviewing orders for safety, consulting with patients and healthcare providers, and supervising technical staff in a busy hospital setting.

The position requires a PA-appropriate license, a Bachelor's degree in Pharmacy, and preferably a PharmD.

Qualifications

  • Pharmacist license in Pennsylvania (RP) upon hire.
  • Bachelor's degree in pharmacy required with eligibility for PA licensure.
  • Doctorate in pharmacy (PharmD) preferred.

Responsibilities

  • Oversees clinical activities of assigned areas to ensure delivery of safe and effective patient care.
  • Maintains patient records, prescription records, and other documentation according to LVPS.
  • Supervises activities of IV technicians including verifying labels, documenting accuracy, and compounding technique.
  • Acts as contact person and resource to physicians, nurses, discharge planners, case managers, etc. regarding treatment options.
  • Monitors patient records to ensure proper patient education and compliance with OBRA regulations.
  • Coordinates activities of the patient care plan including updates and revisions.
  • Maintains adequate inventories of products needed for daily operations.
  • Ensures timely deliveries by checking supplies and coordinating with colleagues.

Summary

Fills and dispenses prescription orders in accordance with the laws of the Commonwealth of Pennsylvania and according to the latest standards of pharmacy practice. Reviews all medication orders for appropriateness and consults with patients and other healthcare providers. Supervises all technical staff and maintains adequate inventory of medications and supplies.

Job Duties
  • Oversees clinical activities of assigned areas to ensure delivery of safe and effective patient care.
  • Maintains patient records, prescription records, and other documentation according to LVPS.
  • Supervises activities of IV technicians including verifying labels, the accuracy of documentation, ensuring proper product selection, and compounding technique.
  • Acts as contact person and resource to physicians, nurses, discharge planners, case managers, etc. regarding treatment options including medication usage and therapeutic dosage, appropriateness of therapy, potential adverse reactions and contraindications, modifications, or alternatives to therapy.
  • Monitors patient records to ensure proper patient education and ensures compliance with maintaining both clinical and prescription information including counseling of patients as mandated by current OBRA regulations.
  • Coordinates activities of the patient care plan including overseeing patient care activities, updating, and making revisions to the patient care plan.
  • Maintains adequate inventories of products necessary for daily business operations.
  • Ensures deliveries are made timely by checking supplies and medications are accurate all while coordinating with colleagues to ensure delivery times are met.
  • Makes recommendations for therapy order changes to prescribing physician.
Minimum Qualifications
  • Bachelor's Degree in pharmacy
  • DL - Driver's License_PA - State of Pennsylvania Upon Hire
  • RP - Registered Pharmacist - State of Pennsylvania Upon Hire
Preferred Qualifications
  • Doctorate in pharmacy (PharmD)
Physical Demands

Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
