Qualifications

Responsibilities

• Provide patient counseling, drug information, and staff education.
• Review patient profiles, labs, dosing intervals, and therapy combos for safety and effectiveness.
• Maintain accurate medication lists for all patients.
• Consult with providers on therapy recommendations and educate patients on medications and side effects.
• Dispense, compound, enter orders, verify all pharmaceutical activities; repackage and distribute prescriptions.
• Document patient and procedural information; ensure compliance with billing practices and formulary restrictions.

Job Summary

Under limited supervision, performs clinical pharmaceutical care for patients in collaboration with other pharmacists, physicians, other healthcare providers, and patients in an accurate and timely manner.

Responsibilities

Provides patient counseling, drug information, drug therapy consultation, and staff education.

Reviews patient profile and clinical status, labs, dosing intervals, and drug therapy combinations to assure safe and effective medication use.

Ensures maintenance of an accurate medication list for all patients.

Consults with healthcare providers on therapy recommendations as appropriate. Educates patients regarding proper use of medication and expected side effects.

Conducts medication dispensing, compounding, order entry, and verification of all pharmaceutical activities. Repackages and distributes prescriptions; performs compounding of IV, topical, and oral medications in an accurate and timely manner.

Documents patient and procedural information.

Accountable for adhering to all pharmacy billing practices, formulary restrictions, and related policies and procedures to maximize revenue opportunities while ensuring patient safety standards are met.

Reviews and authorizes pharmacy technician activities. Responsible for any delegated act performed by pharmacy technicians. Participates on various committees.

Ensures compliance with the regulatory and practice standards of pharmaceutical care.

Performs other duties as assigned or required.

Pharmacist Level Expectations

Pharmacist II: Involvement in one committee.

Pharmacist III: Involvement in 2+ committees OR clinical specialist in respective area.

Pharmacist IV: In addition to duties of Pharmacist II and III listed above, serves as a source of leadership across departments AND serves as a subject matter expert in at least one area or initiative.

Minimum Education Qualification

A Doctorate degree in Pharmacy (BS for candidates who were licensed under the BS License requirement) from an accredited college program.

Minimum Experience Qualification

Pharmacist I – Non-supervisory, entry level; no experience required.

Minimum Certification Qualification

• Certification: Current State of Alaska Pharmacy License or legal equivalent.
• Advanced Cardiac Life Support is required for Inpatient Pharmacists (on-site only) OR able to obtain within a 4‑month period after hire date.

Preferred Experience Qualification

Inpatient Pharmacist I‑IV: Inpatient preferred experience – residency or 3 years of equivalent clinical experience.

Preferred Certification Qualification

Board certification or an advanced degree.

Additional Requirements

All employees must don appropriate personal protective equipment (PPE) if exposure to hazardous materials, sharp objects, or infectious diseases is present. Employees must also operate within the department’s appropriate engineering controls such as negative pressure storage rooms and compounding hoods.
