Job summary

Yakima Valley Farm Workers Clinic seeks a Float Pharmacist to support multiple clinics across Oregon. You will integrate into a team-based setting, manage medications, perform clinical duties, and travel regularly among Salem, Woodburn, and Portland with occasional coverage in Astoria.

Qualifications

  • Bachelor's Degree in Pharmacy and current WA State Pharmacist License.
  • PharmD degree and bilingual (English/Spanish) preferred.
  • Drug testing prior to employment and during employment.

Responsibilities

  • Provide patient care services including DRUG Utilization Review and patient counseling.
  • Oversee pharmacy personnel and ensure licensure scope compliance.
  • Prescribe or modify care plans via collaborative agreements where allowed.
  • Maintain records, inventory, and compliance with laws.

As a pharmacist at Yakima Valley Farm Workers Clinic, you are truly integrated into a team-based healthcare organization working to the top of your licensure. Pharmacists are involved in all aspects of patient care and have access to patient charts, labs, medications, and diagnoses. With the ability to prescribe or modify care plans via collaborative agreements, our goal is to ensure patients receive the best and most complete, cost-effective medications and care possible. Aside from traditional outpatient dispensing, pharmacists provide clinical support to medical providers via chronic refill authorization, therapeutic interchanges, and prior authorization completions. Additionally, there is an opportunity for clinical visits managing diabetes, hypertension, anticoagulants, and asthma, amongst other chronic conditions, with direct face-to-face or telehealth patient care.

Please note this is a float position, and regular travel is required to several pharmacy locations, spanning from Salem, Woodburn, and Portland, Oregon, with potential occasional coverage at the Astoria location.

  • A Pharmacist passionate about serving underserved communities

  • $146,000 - $168,000 yearly compensation DOE with the ability to increase for highly experienced candidates + float differential

What You’ll Do:
  • Provide patient care services as assigned, which include but are not limited to:
    • Drug Utilization Review
    • Patient Counseling
    • Medication Therapy Management
    • Perform Collaborative Drug Therapy Management or other post-diagnostic disease state management, pursuant to a valid agreement
    • Prescribe drug or device, as authorized by statute, according to Drug Therapy Management Protocols
    • Order, interpret, and monitor laboratory tests
    • Receive or transfer of a prescription
  • Provide direct oversight of pharmacy personnel, ensuring work is within the scope of practice and duties of their licensure
  • Ensure the security of the pharmacy area, including the provision of adequate safeguards against theft or diversion of prescription drugs
  • Ensure all records and inventories are maintained in accordance with state and federal laws
  • Ensure new prescriptions, refills, and drug orders are correctly dispensed according to the provider’s authorization and YVFWC policies and procedures
  • Maintain active licensure and pharmacological knowledge by attending educational workshops, reviewing professional publications, establishing professional networks, and participating in professional societies
Qualifications
  • To be considered for this position, you must complete a Bachelor's Degree in Pharmacy and have a current Washington State Pharmacist License.
  • A Pharm D Degree and Bilingual (English/Spanish) speaking skills are preferred
  • Drug testing: This position requires testing for controlled substances before employment and will be subject to further testing throughout employment.
