Job summary

CP01 Carilion Medical Center in Roanoke, VA is seeking a Clinical Pharmacist for Day/Evening shifts. In this role, you will provide clinical consultation, educate patients and healthcare providers, and ensure accurate medication dispensing.

Qualifications

Overview

Position: Pharmacist, Clinical Staff – Day/Evening (Flex/PRN).

Employment Status: Variable / Flex Shift; Hours: Variable.

Location: 1906 Belleview Ave SE, Roanoke, VA.

Responsibilities

• Provide clinical consultation for other health‑care providers
• Educate patients, patients’ families and health‑care staff on medication use
• Review, verify, and process medication orders for accuracy
• Prepare and dispense medications, including sterile and non‑sterile compounding per SOP
• Participate as an active member of interdisciplinary care teams and pharmacotherapeutic decision making
• Adhere to collaborative practice protocols
• Provide virtual pharmacy consults to Carilion Clinic ambulatory offices
• Assist with drug information and evidence‑based medicine support
• Respond to patient emergencies when required
• Ensure timely delivery of medications to patients and providers
• Educate and train healthcare providers, students, residents, patients, and families
• Provide feedback to pharmacy students and residents
• Maintain competency in all technology used by the Department of Pharmacy
• Support clinical pharmacy initiatives of the institution
• Maintain compliance with statutes, laws, regulations, standards and policies governing pharmacy practice
• Facilitate age‑specific pharmaceutical care to patients

Qualifications

• Graduate of an accredited College of Pharmacy (ACPE)
• Preferably completed a Pharmacy Practice Residency or equivalent hospital experience

Licensure, Certification and Registration

License to practice Pharmacy in the Commonwealth of Virginia required within 90 days of hire. The pharmacist may not perform duties restricted to a licensed pharmacist until a valid license is obtained. Failure to obtain the license within 90 days may result in separation.
