UCHealth Anschutz Inpatient Pavilion in Aurora, CO seeks a Pharmacist Clinical Specialist for the General Hematology Clinic to coordinate pharmaceutical care for patients with benign hematologic disorders.

You will collaborate with hematology providers, manage disease- and drug-specific therapies, provide education to patients and caregivers, and support safety, efficacy, and cost-effective care in a fast-paced oncology setting.

Pharmacist Clinical Specialist - General Hematology Clinic

Location: UCHealth Anschutz Inpatient Pavilion, Aurora, CO

Department: UCH Amb HOPD Pharmacy (Oncology)

Work Schedule: Full Time, 80.00 hours per pay period (2 weeks)

Shift: Days

Pay: $62.19 - $87.07 / hour. Pay is dependent on applicant's relevant experience

This position is an onsite role and does not offer a hybrid or remote option

Summary

Coordinates comprehensive pharmaceutical and patient care for patients with benign hematologic disorders. Serves as the pharmacy clinical specialist for the benign hematology practice and works collaboratively with hematology providers and the interdisciplinary care team to optimize medication therapy, improve patient outcomes, and support safe, evidence-based, and cost-effective care. Provides cross-coverage for malignant hematology and cellular therapy clinic pharmacists as needed.

Responsibilities

• Develops, implements, and manages evidence-based medication therapy for patients with benign hematologic disorders in collaboration with physicians, advanced practice providers, nurses, and other members of the interdisciplinary team.
• Provides comprehensive medication management for patients receiving disease- and drug-specific therapies, including assessment of treatment appropriateness, efficacy, safety, adherence, drug interactions, monitoring requirements, and supportive care needs.
• Identifies high-risk patients and implements evidence-based interventions to optimize medication-related outcomes and prevent treatment-related complications.
• Provides patient and caregiver education regarding medications, treatment plans, administration, adverse-effect management, adherence, monitoring, and supportive care.
• In conjunction with a patient's healthcare provider, provides, delegates, and coordinates patient care, which may include medication initiation and adjustment, laboratory monitoring, point-of-care testing, and follow-up care within applicable scope of practice and collaborative practice agreements.
• Serves as a medication and therapeutic resource for the benign hematology team and mentors pharmacy staff and learners in areas of clinical expertise.
• Collaborates with specialty pharmacy, medication access, nursing, providers, and other stakeholders to facilitate timely initiation and continuation of therapy and address medication access barriers.
• Develops and leads quality, safety, workflow, and affordability initiatives at the clinic, service-line, and department levels.
• Documents clinical interventions and patient outcomes to support continuity of care, quality improvement, and evaluation of pharmacy services.
• Participates in development and implementation of clinical guidelines, treatment pathways, protocols, order sets, and other medication-use processes relevant to hematology practice.
• Participates in research, quality improvement initiatives, professional development activities, and interdisciplinary committees to evaluate and advance standards of hematologic care.
• Provides cross-coverage for malignant hematology and cellular therapy clinical pharmacy services as needed.

Requirements

• Doctor of Pharmacy from an accredited school of Pharmacy.
• State licensure as a Pharmacist and any relevant life support certification as determined at position level.
• 5 years specialty experience OR PGY1 or clinical fellowship completion with 3 years' pertinent work experience OR Pertinent PGY2 completion.

Preferred

• PGY2 residency training in oncology or another relevant specialty.
• Board certification in oncology pharmacy or another applicable specialty.
• Experience providing direct patient care in benign hematology, hematology/oncology, or an ambulatory hematology practice.
• Experience with collaborative drug therapy management, patient education, specialty medications, and medication access processes.
