Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Basic Requirements:

Duties

• The pharmacist is a licensed professional with knowledge and proficiency in pharmacy practice in a variety of pharmaceutical services
• The pharmacist will function in a hybrid capacity that includes both distributive and clinical responsibilities
• Clinical responsibilities may include pharmacotherapy - pharmacokinetics - anticoagulation - and antimicrobial stewardship
• Distributive functions include but are not limited to reviewing - interpreting - and verifying medication orders - processing - and filling medication orders - PDMP checks - dispensing - compounding - and preparing medication orders
• The primary function of a pharmacist is to assure the safe and appropriate use of medications and be an advocate of rational drug therapy through the following: evaluation of the appropriateness of drug therapy based on patient specific factors - evaluation - dispensing and providing medications - and drug information
• Responsible for the safe and appropriate distribution or dispensing of medication
• Reviews all medication orders for appropriateness - drug selection - dosage - route of administration and the amount - prior to dispensing
• Ensures all medication orders are entered into the electronic patient medical record
• Completes patient chart reviews - evaluates pertinent laboratory data - drug-drug - and drug-nutrient interactions - monitors for adverse drug effects - and screens for allergies
• Provides pharmacokinetic consultation - including dosing vancomycin and aminoglycosides - in accordance with local policy
• Provides anticoagulation consultation including dosing and monitoring of anticoagulants - in accordance with local policy
• Review patient medication regimens for clinical effectiveness - drug selection - dosing - contraindications - side effects - potential drug interactions - and therapeutic outcomes as required Communicates findings
• with prescribers and provides appropriate alternatives to current treatment plans as needed
• Obtains medication histories (Med Recon) from patients and summarizes significant findings in the medical record and/or to the provider as appropriate
• Monitor and report drug errors - adverse drug reactions - allergies - and patient compliance issues Documents findings per facility procedures
• Assists with administrative paperwork as requested by supervisor(s)
• Assists in training new staff who will be assigned to perform WHEN hour duties
• Ensures staff continue to follow proper processes and procedures
• Provides any concerns regarding training - performance - and competencies to the appropriate supervisor
• Responsible for day-to-day operations in accordance with established priorities and assistance from supervisors
• Provides guidance and technical direction necessary for accomplishing the work
• Assists with oversight of daily operations and facilitates workflow to ensure operations run smoothly
• Assesses workload volume throughout the evening shift - identifying backlogs and makes staffing adjustments as needed to complete assigned tasks and meet goals
• Serves as an authority on drug usage - interactions - over-dosages - and compliance - to medical staff - nursing staff - technicians - and students
• Uses appropriate references to research drug information
• Has a good working knowledge of information resources - both written and electronic
• Maintains a current knowledge of therapeutics and disease management
