MedPlus Infusion Pharmacy seeks a Clinical Pharmacist to join its home infusion team. Prior home infusion experience is preferred; IV certification is advantageous but not required. The role centers on sterile IV compounding, TPN formulations, and ensuring safe, effective drug therapy.

Responsibilities include admissions collaboration with nursing, monitoring outcomes, documenting clinical activities, and verifying compounded medications for delivery. Compensation is based on experience.

Qualifications

• Experience and knowledge in sterile IV compounding per USP 797 guidelines.
• Ability to facilitate new patient admissions with nursing and intake staff.
• Proficiency in monitoring safety and efficacy of drug therapy.
• Experience documenting medication profiles, progress notes and care plans.
• Ability to check compounded meds and troubleshoot infusion devices.
• Bachelor’s degree in pharmacy; clinical experience acceptable in lieu of PharmD.

• Facilitates new patient admissions through collaboration with nursing and intake staff. Receives, verifies, reviews and documents all new patient orders. Establishes parameters for monitoring the safety and efficacy of drug therapy.


• Monitors the outcomes from drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information. Communicates the results of therapeutic drug monitoring activities to the physician, nurse and other appropriate persons.


• Completes and maintains appropriate documentation related to medication profiles, progress notes, assessments, care plans and all other clinical activities, in accordance with applicable state laws, regulatory agencies, Company policy and procedure, and accreditation standards.


• Checks compounded medications to ready for delivery. Programs and troubleshoots infusion control devices as indicated.


• Bachelor’s degree in pharmacy and clinical experience is acceptable in lieu of PharmD.
