Job summary

UPMC Chartwell is seeking a full-time Pharmacist to join a dedicated team. The Pharmacist will ensure safe, effective and cost-efficient pharmacotherapy, support medication-use systems, mentor students, and lead quality initiatives across departments.

The role requires ACPE-accredited education, a PharmD or BS in Pharmacy with current licensure, and entry-level candidates with pharmacist experience preferred.

Qualifications

• ACPE-accredited College of Pharmacy education required.
• BS in Pharmacy or PharmD and current state licensure required.
• Entry level with pharmacist experience preferred.

Responsibilities

• Interpret and evaluate orders to dispense medications for inpatients and outpatients.
• Identify and report adverse drug reactions in a timely manner.
• Maintain formulary, promote rational drug therapy and handle therapeutics substitutions.
• Review patient medication records for allergies and interactions, verify orders.
• Supervise pharmacy technicians and assistants as part of the team.
• Adhere to regulatory standards and hospital quality policies in care delivery.
• Communicate with physicians and nurses to clarify orders promptly.
• Provide drug information and training to healthcare professionals and students.
