The Pharmacist-Ambulatory (Heart & Valve Center) is a full-time, on-site role based in Lexington, KY, providing advanced ambulatory care pharmacotherapy services to patients with cardiovascular conditions. This role collaborates closely with cardiologists, advanced practice clinicians, nurses, and other care team members to optimize medication therapy, support evidence-based treatment plans, and improve clinical outcomes for heart and valve patients. Day-to-day responsibilities include conducting comprehensive medication reviews, managing complex drug regimens, providing patient and family education on medications and disease states, and monitoring for efficacy, safety, and adherence. The pharmacist participates in multidisciplinary clinics, contributes to care pathway development, assists with transitions of care and medication reconciliation, and supports quality improvement initiatives and population health efforts.
