Job summary

PharMerica is hiring a full-time Pharmacist in Horsham, PA for a non-retail, closed-door pharmacy serving long-term care and senior living clients.

The role emphasizes clinical medication management, collaboration with physicians and facility staff, and adherence to regulatory standards, with a Monday–Friday 3:00pm–11:30pm schedule and rotating weekends.

Qualifications

  • Licensed pharmacist in the state of practice.
  • Pharmacist with long-term care experience preferred.
  • Knowledge of CMS/DEA/FDA guidelines and regulatory standards.
  • Experience with EMR and pharmacy information systems.
  • Strong clinical judgement and communication skills.

Responsibilities

  • Evaluate medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
  • Provide clinical recommendations to physicians and facility staff regarding medication therapy changes and optimizations.
  • Maintain up-to-date patient/resident profiles, including medical history, lab results, and current medications.
  • Ensure laboratory tests required for monitoring high-risk medications are ordered, reviewed, and communicated to the healthcare team.
  • Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
  • Document all communications and clinical interventions in the patient’s medical record.
  • Implement and enforce policies and procedures for the safe procurement, storage, administration, and disposal of medications and biologicals.
  • Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management.
  • Report medication-related errors and adverse drug events to the Pharmacy Manager as required.
  • Educate and support facility/agency staff on safe and effective medication handling procedures.
  • Perform other pharmacy-related tasks and responsibilities as assigned.

As a Pharmacist you will

  • Document all communications and clinical interventions in the patient’s medical record, including interactions with families and caregivers.

Qualifications
  • Bachelor’s Degree in Pharmacy, Pharm D preferred
  • One year experience in long term care desired
  • License/Certification(s): current and valid pharmacist license in the state of practice
  • Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
  • Proficiency in electronic medical records and pharmacy information systems
  • Strong clinical judgement, and effective written and verbal communication
