Job summary

PharMerica in Washington, NC is seeking a full-time Pharmacist for a closed-door, long-term care setting. You will review orders, advise care teams, and maintain patient profiles to ensure safe, effective medication management.

Responsibilities include monitoring high-risk therapies, collaborating with physicians and nurses, and ensuring regulatory compliance. A valid pharmacist license and LTC experience are preferred. Benefits include comprehensive medical, dental, 401(k), and other programs.

Qualifications

• Bachelor’s Degree in Pharmacy or Pharm D; licensure in the state of practice.
• 1 year of long-term care experience desired.
• Current pharmacist license in state of practice.
• Knowledge of CMS, DEA, FDA guidelines and regulatory standards.
• Proficiency with electronic medical records and pharmacy information systems.
• Strong clinical judgement and effective communication.

• Evaluate physician medication orders for appropriateness, dose, interactions, and route.
• Provide clinical recommendations to physicians and facility staff on therapy changes.
• Maintain up-to-date patient profiles with medical history, labs, and meds.
• Ensure lab monitoring for high-risk medications is ordered and reviewed.
• Collaborate with physicians, nurses, and staff to support care.
• Document communications and interventions in the patient records.
• Enforce policies for safe procurement, storage, and disposal of medications.
• Ensure compliance with state and federal pharmacy regulations.
• Report medication errors to Pharmacy Manager.
• Educate facility staff on safe medication handling.
• Perform other pharmacy-related tasks as assigned.

As a Pharmacist you will:

• Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
• Provide clinical recommendations to physicians and facility staff regarding medication therapy changes and optimizations.
• Maintain up-to-date patient/resident profiles, including medical history, lab results, and current medications (prescription, OTC, and home remedies).
• Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered, reviewed, and communicated to the healthcare team.
• Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
• Document all communications and clinical interventions in the patient’s medical record, including interactions with families and caregivers.
• Implement and enforce policies and procedures for the safe procurement, storage, administration, and disposal of medications and biologicals.
• Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management.
• Report medication-related errors and adverse drug events to the Pharmacy Manager as required.
• Educate and support facility/agency staff on safe and effective medication handling procedures.
• Perform other pharmacy-related tasks and responsibilities as assigned.

• One year experience in long term care desired
• License/Certification(s): current and valid pharmacist license in the state of practice
• Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
• Proficiency in electronic medical records and pharmacy information systems
• Strong clinical judgement, and effective written and verbal communication
